What enrolling involves, before you commit to any of it.
There is more paperwork in licensed child care than most parents expect, and none of it is a surprise I want you to discover on your first morning. All of it is below, so you know what you are agreeing to.
You do not need any of this yet. If you are still deciding, start with a visit.
Already with me? Every form below is in the Family Area, behind your login, along with the menus and the calendar.
What you sign when you enroll
Seven documents. Two of them are mine, and five are required by the State of New Hampshire.
Mine
- Business Policy and Contract. Hours, holidays, sick days, what you provide and what I provide, and the two-week trial period. The plain-language version of most of it is on the questions page.
- Financial Agreement. Your weekly rate, your arrival and pick-up times, and how payment works.
Required by the state
- Child Care Registration and Emergency Information. Who your child is, who I call, and who may collect them.
- Personal Information Form. Your details, your employers, your doctor, and any allergies or conditions.
- Emergency Medical Authorization. Your consent for your child to get emergency medical care if I cannot reach you first.
- Parent Authorization for Licensing Visits. Your consent for the state’s unannounced licensing visits, which happen yearly or more often.
- Electronic Monitoring Authorization. Your consent to the electronic monitoring in the day care space.
Two more, and they come from other people
These are the ones worth starting early, because you are waiting on somebody else for both.
- An up-to-date immunization record. Required by licensing. If you would rather not immunize, there is a waiver with its own criteria, and a waiver on file counts in place of the record.
- A Child Health Form, signed by your child’s physician, saying your child is able to attend day care.
The permissions you choose
These are yours to grant or refuse, one at a time.
- Permission to photograph
- Transportation, for field trips
- Sunscreen and insect repellent
- Swimming, for the summer pool
- Accepting or declining formula
What you get back
- The weekly menus, so you know what your child is eating
- The holiday and vacation calendar, published each January and shareable to your own phone
- A login to the Family Area, where all of it stays current
